               CASE HISTORY 

Patient details 
A 62 years old male, named Andhraiah,occupation farmer,Resident of mothkur presented with

Chief complaint 
Swelling   in the Neck region since one month. 

History of present illness 
Patient was apparently asymptomatic one month ago then he later had swelling in the  Neck region one month back relieved on medication.  And then he had swelling in the buttock region. 

History of past illness  
Diabetic  since 20 years 
Hypertension: No 
CVD: No 
Epilepsy: No 
Tuberculosis: No 

Family history 
No similar  complaints

Personal history 
Diet: mixed 
Appetite: Normal 
Sleep: Normal 
Micturition: Normal  
Bowel movements: Normal 
Addictions: No  addictios
             

General examination 
Built and nourishment: Normal 
Pallor:No
Icterus: No
Cynosis: No
Clubbing: yes
Lymphadenopathy: No cervical lymphadenopathy 
Oedema: No 
Provisional diagnosis:
               Gluteal abscess
